What is protruding mouth?


Protruding mouth is “bimaxillary dentoalveolar protrusion” in medical terminology, which is characterized by unsightly, forward positioned profile line of protruding lips (commonly with receding chins) when compared to the ideal reference line of lower face.


Mild protruding mouth cases may be improved by mere orthodontic treatment.

However, Moderate or severe skeletal protrusion of the alveolar (gum) bone itself must be treated by surgery, i.e. ASO(Anterior Segmental Osteotomy)

What is ASO?


ASO(Anterior Segmental Osteotomy) surgery is performed for the patients with protruding mouth of moderate to severe grade.


ASO is a surgery that moves the maxillary and mandibular alveolar bone & six teeth complex backwards at once following extracting the (upper and lower) four premolar teeth, under general anesthesia. The surgery time of ASO is only about one hour or less.

  • 1. What is protruding mouth surgery (ASO)?

    Protruding mouth surgery, also known as ASO (Anterior Segmental Osteotomy), is a surgical procedure that repositions the front portions of the upper and lower jaws to correct an excessively forward lip and gum position. It’s often used to improve facial balance, side profile, and lip closure.

  • 2. Who is a good candidate for ASO surgery?

    Ideal candidates have moderate to severe bimaxillary dentoalveolar protrusion—meaning the upper and lower jaws and teeth are pushed forward. Common signs include lips that do not close naturally, a gummy smile, and a protruding philtrum.

  • 3. Is orthodontic treatment alone enough to fix a protruding mouth?

    In mild cases, orthodontics may help. However, for skeletal-level protrusion, orthodontics cannot move the underlying bone structure back. In such cases, ASO surgery is necessary for full correction.



  • 4. How is ASO surgery different from double jaw (two-jaw) surgery?

    ASO focuses on moving only the anterior (front) segments of the jawbone, whereas double jaw surgery involves repositioning the entire upper and lower jaw. ASO is less invasive, has shorter recovery, and avoids intermaxillary fixation (rubber bands holding jaws shut).

    Typically, ASO surgery takes around 1 hour. If chin surgery (genioplasty) is performed together, it may take an additional 20–30 minutes.

  • 6. Will I be able to speak and eat after surgery?

    Yes. Since ASO does not require jaw fixation with rubber bands, patients can speak, drink, and breathe normally right after surgery. A soft diet is recommended during early recovery.

  • 7. How long is the recovery period?

    Most patients can return to daily activities within 3–4 days. Swelling and mild bruising may last for 1–2 weeks. Final results are typically visible after 1–2 months.



  • 8. Are there any scars after ASO surgery?

    No. All incisions are made inside the mouth, so there are no visible external scars.

  • 9. Can ASO surgery be combined with other procedures?

    Yes. ASO is often combined with genioplasty (chin surgery) to improve overall facial harmony. Some patients also undergo eye or nose procedures for full-profile refinement.

  • 10. Is the result permanent?

    Yes. Once the bone segments heal in their new position, the result is permanent. With proper care and follow-up, there is no risk of the bones returning to their previous position.
